Client Liability Waiver

I acknowledge that participating in fitness programs involves inherent risks, including physical injuries or medical conditions.*

I voluntarily assume all risks associated with participating in fitness programs.*

I release Tianah's Fitness Training, its owner, employees, and agents from any liability, claims, or actions of any kind arising from my participation in fitness activities.*

I certify that I am in good health and have disclosed all relevant medical conditions.*

In the event of an emergency, I authorize Tianah's Fitness Training to obtain medical treatment on my behalf and agree to assume all associated costs.*

I understand that I am solely responsible for supervising my child(ren) during workouts and release Tianah's Fitness Training from any liability related to their presence.
